1996 – 2019
Putnam – Patrick D. Falkowski, 23, of Lyon St., passed away unexpectedly on Saturday, November 16, 2019. Born in Worcester, MA on February 14, 1996, he was the son of Kathleen A. (Hultberg) Falkowski of Putnam, CT and Michael S. Falkowski of Tampa, FL.
Patrick was a 2014 graduate of Woodstock Academy where he was the Captain of the Football Team, and an All-State Nominee. He received Honorable Mention at his Senior year football awards ceremony. Patrick was also a member of the WPTP Youth Football League in the town of Putnam, CT. He was a member of the Cross-Country team at Woodstock Middle School. He enjoyed playing football, basketball, and loved watching the New England Patriots. He was a hard-working, devoted student and athlete loved by everyone. Patrick was a very sociable, kind, caring, laid back kind of guy who greeted everyone with a warm smile. He was always there to help anyone in need. He worked at Aubuchon Hardware in Webster, MA for three years. He recently worked at U.S. Button Corporation for a short time. He was a hard-working young man who always put 100% effort into everything he did.
In addition to his parents, Patrick is survived by his two brothers, Matthew Falkowski of Putnam, Michael Pescosolido of Southbridge, MA, and his sister Jenna Falkowski, of Tampa, FL. He is also survived by his paternal grandmother Patricia LaRosa, of Philadelphia, PA, and predeceased by maternal grandmother, Barbara Lee Hultberg, of Oakdale, CT and predeceased by maternal grandfather, Clement M. Hultberg, of Auburn, MA, He also leaves behind his four Aunts: Susan Hultberg Pratt, of Florida, and Karen Hultberg Fenuccio, of Lunenburg, MA, Patricia Dimitt, of VA, Deanna McClintic, of PA, and an Uncle, David Hultberg, of East Brookfield, MA. He is also survived by his many cousins, Michelle Hippert, of Charlton, MA, Leandra Carey, of Auburn, MA, Ryan Carey, of Delray Beach, FL, Tyler Carey, of Amherst, MA, Nicholas Fenuccio of Keane, NH, and Joey Fenuccio, of Keane, NH, and many cousins from Pennsylvania, and the Virginia area. He was also loved by many friends from Woodstock Academy and the Connecticut area.
Relatives and friends are invited to visit with Patrick’s family from 11:00 a.m. to 1:00 p.m. on Tuesday, December 3, 2019 in the Gilman Funeral Home & Crematory with a funeral home service taking place at 1:00 p.m.in the funeral home.
